School Health Requirements For Attendance 2022-23 The school year begins Monday, August 10, 2022.
Prepare now! Have your children seen for an annual physical exam and review their immunizations. A physical exam, signed and stamped by a licensed provider and dated within one year prior to registration, is required for each student initially entering a Florida school. This is proof that body systems (heart, skin, lungs) were examined. DOH-Pasco will NOT be providing school physicals for the 2022-23 school year. Up-to-date immunizations are required, and all immunization dates must be on Florida Certificate of Immunization (DH680). Contact your physician or the DOH-Pasco for the form. DOH-Pasco clinics provide free childhood immunizations. Appointments are required.

Kindergarten and Grades 1-6:
5 DTP (for KG, last dose on/after 4th birthday)
4 Polio (last dose on/after 4th birthday)
2 MMR
3 Hepatitis B
2 Varicella for KG and grades 1, 2, 3, 4, 5 and 6
Grades 7-12:
5 DTP
4 Polio (last dose on/after 4th birthday)
2 MMR

1 TDaP
2 Varicella for grades 7, 8, 9, and 10
1 Varicella (chicken pox) for grades 11 -12
